DaVita Senior Manager, Health Enhancement in El Segundo, California


Responsible for the overall planning, implementation, and evaluation of health enhancement/education program serving patients for the HealthCare Partners network. This position is also responsible for the development of the patient education components of care for all population and disease management programs.


  • Responsible for leadership and coordination of the identification, planning, implementation, and evaluation of disease management and population management programs as they relate to the Health Enhancement Department.

  • Supervises daily operations, staff, contractors, programs and services.

  • Participates in the integration of Health Enhancement interventions into existing ambulatory care programs and services.

  • Participates in the development of outcome measurement/benchmarks in regard to disease and population management programs.

  • Coordinates strategic planning efforts to ensure that health enhancement requirements are fulfilled and needs are met according to health plan requirements.

  • Assures maintenance and continued improvements of all existing health enhancement programs including oversight of contracts/teaching agreements; approve network scheduling.

  • Oversees marketing activities for Health Enhancement Services (patient calendars, health education brochures, patient education literature, etc. ) to ensure continuity of care and communication to providers and patients.

  • Chairs (in conjunction with appropriate medical director) the appropriate health enhancement committee meetings and other appropriate committees; represents or determines representation of department on other committees, task forces, meetings, etc.

  • Responsible for development of department business plan and labor, no-labor, and capital budgets.

  • Provider oversight of systems for tracking/reporting utilization and dashboard development

  • Functions as a liaison to community/professional organizations as designated.

  • Assumes other duties as delegated.



  • Bachelor’s degree from a four-year college and/or a professional certification requiring formal education beyond a two year college in a related field.


  • Master’s degree in health education, public health MHA or MHSA, or related field.



  • Over 5 years and up to and including 7 years of experience.

  • Knowledge of population based healthcare, CQI/TQM principles, and databases; experience in developing and implementing health enhancement programs, outcome studies.


  • RN/RD license.

  • Three to four years management experience.
